How to track body composition without a DEXA scan

Here's the secret the scan-selling industry won't lead with: for tracking change, which is what almost everyone actually wants, a consistent home protocol beats an occasional DEXA. Scans excel at absolute snapshots; trends live in cheap, repeatable measurements taken the same way every time. This is the protocol.
The whole system runs on two trend lines and one decision table. Weight tells you the direction of total mass; waist tells you the direction of central fat. Read together they identify which of five situations you are in, and each situation has a different correct response, including, in two cases, doing nothing at all.

- Weight and waist read together identify what is happening; either one alone is ambiguous and routinely leads people to change a programme that is working.
- Weigh daily or three mornings a week and read only the seven-day average: a single morning's weight is mostly food, fluid and glycogen.

The monthly kit: scale, tape, camera
Weight: weigh daily if it doesn't stress you (first thing, after bathroom, before food) and read only the 7-day average: single days are water noise. If daily weighing isn't good for your head, three mornings a week averages nearly as well, and skipping the scale entirely in favour of tape-only is a legitimate choice.
Tape, monthly: waist at the navel, hips at the widest point, plus neck, which unlocks our Navy body fat estimate. Add chest, upper arm and mid-thigh if you're training for muscle; they show growth the scale hides. Same morning of the month, same landmarks, same normal exhale.
Photos, monthly: front, side and back, same spot, same lighting, same time of day, phone propped at the same height. Photos catch recomposition (fat down, muscle up at identical scale weight) better than any number, and comparing month one to month six is frequently the moment people realise their 'stalled' program worked.
Total outlay: a five-dollar tape, a scale you probably already own, and a phone. Total time: about ninety seconds a morning for the weight, and twenty minutes once a month for everything else. That is the entire capital and labour cost of a body composition tracking system that outperforms quarterly scanning for the purpose most people actually have.
The monthly protocol, step by step
Put the monthly session in the calendar as a repeating appointment on a fixed date: the first Saturday, the first of the month, whatever you will actually keep. The value of the whole system comes from the conditions being identical, and the fastest way to lose that is to measure whenever you happen to remember.
- Wake, use the bathroom, and measure before eating or drinking anything.
- Weigh first, on the same scale, on the same hard floor, with nothing on.
- Waist: three passes at the same landmark, at the end of a normal exhale. Record all three, then the average.
- Hips: three passes at the widest point of the buttocks, feet together.
- Neck: three passes just below the larynx, tape sloping slightly down at the front.
- Optional, if you are training for muscle: chest, flexed upper arm, mid-thigh and calf.
- Photos: front, side and back, same spot, same light, phone at the same height on the same shelf.
- Enter everything in the log before you leave the room, including anything unusual about the week.
- Compute the derived numbers (waist-to-height, waist-to-hip, the Navy body fat estimate, FFMI) and file them beside the raw measurements.
- Do not compare against last month yet. Compare against the last three months, once you have three.
Turning measurements into conclusions
The power move is combining trend lines. Weight flat + waist down = recomposition; celebrate, change nothing. Weight down + waist flat = the loss is coming disproportionately from lean mass; raise protein toward 2.2 g/kg and check your deficit isn't too deep. Weight up + waist flat-or-down while lifting = muscle gain proceeding as designed. Weight up + waist up = a real surplus, decide whether it's the plan or a drift. Every one of those verdicts comes from ten dollars of equipment.

Expect the absolute values to carry their stated error bands; expect the direction of change to be honest, because your personal measurement bias subtracts itself out of any comparison.
Laid out as a table, the logic is easier to apply at six in the morning than it is to recall from prose. The 'weight' column always means the seven-day rolling average across at least three weeks, and the 'waist' column always means the monthly averaged reading; comparing single days to single days reproduces the noise this whole system exists to filter out.

A worked three-month log
Here is what the system looks like in practice for a man of 178 cm with a 38 cm neck, eating in a moderate deficit and lifting three times a week. The body fat column is the Navy estimate recomputed from his own tape readings each month; nothing in it comes from a scan.
The interesting month is month two. The scale barely moved (eight hundred grams over four weeks, which is inside the noise of a seven-day average), and this is exactly the point at which people conclude the diet has stopped working and cut their calories again. The waist says otherwise: a centimetre and a half is well above technique error and represents real change. The correct response to month two was to change nothing, and month three confirms it.

Why the scale moves overnight
A kilogram of fat stores roughly 7,700 kilocalories, so gaining or losing one overnight would require an energy imbalance no one achieves by accident. What moves is water, glycogen and the physical contents of the digestive tract. Every gram of stored glycogen binds around three grams of water, so a high-carbohydrate day can add most of a kilogram that has nothing to do with fat. Sodium shifts fluid within hours. A large meal is still physically inside you the next morning.
This is not merely a plausible story; it has been measured. When researchers tracked two-week weight changes in free-living adults and measured what the change was actually made of, the average fluctuation of about a quarter of a kilogram was roughly 84% fat-free mass, with an implied energy density around 2,380 kilocalories per kilogram rather than the 7,700 figure people apply to it. Over two weeks, most of what the scale reports is not fat, and treating it as if it were leads to exactly the wrong adjustments.
The practical consequences are two. First, never act on a single morning's weight, and be sceptical of week-to-week differences smaller than about half a kilogram. Second, when converting a rate of weight change into a calorie adjustment, use a longer window, three to four weeks minimum, because the 7,700 kcal per kilogram conversion only starts to behave once the water noise has averaged out.
When a scan is actually worth it
Once, as calibration, if you care about absolute numbers: a single DEXA tells you your offset from the tape estimate, and that offset travels with you for years. Beyond that, scans earn their cost mainly at the extremes: competitive physique athletes near stage weight, clinical monitoring of muscle loss, research. For everyone else, the money buys more progress as six months of protein powder than as quarterly scans that mostly re-measure hydration.
There is one exception worth naming, because home methods genuinely cannot substitute for it. If the question is about muscle rather than fat (age-related muscle loss, recovery of a limb after injury or surgery, or monitoring during a medical condition or medication that affects lean tissue), a DEXA reports regional lean mass, and nothing you can do with a tape does. That is a different question from 'is my diet working', and it deserves a different tool.
If you do book a scan, book it properly. Go in the same hydration state you would for any other measurement: morning, fasted, before training, having not carbohydrate-loaded the day before. Take your tape measurements the same morning so the calibration is against a matched reading. And ask the clinic which machine and software version they use, because scans from different systems are not directly comparable, which makes a second scan somewhere else considerably less useful than a second scan in the same room.
When the trend says nothing is happening
Four to six weeks of flat weight and flat waist is the situation the whole system exists to resolve, and it has exactly three explanations. The first is that intake is higher than recorded. This is overwhelmingly the most common one and it is not a character failing: measured under-reporting of food intake in free-living adults is large and systematic, cooking oils and drinks are the usual culprits, and a fortnight of weighing everything rather than estimating usually finds the gap.
The second is that expenditure has fallen. A smaller body costs less to run, and a sustained deficit reduces spontaneous movement as well, which means the deficit you calculated three months ago is smaller than it was. The fix is to recalculate maintenance from current body weight rather than to cut further from an out-of-date number.
The third is that the measurements are hiding a real change. This is where photos earn their place: simultaneous fat loss and muscle gain can hold weight, waist and even a tape-based body fat estimate almost still while appearance changes noticeably. If the numbers are flat and the month-one and month-three photographs are not, believe the photographs and change nothing.

How often should I weigh myself?
Daily is ideal if it does not affect your relationship with food, because more data points make a cleaner seven-day average, and the average is the only thing worth reading. Three mornings a week gives nearly the same quality of average with a third less exposure to a number that moves for reasons unrelated to fat. Weigh at the same time under the same conditions: first thing, after the bathroom, before food or drink, on the same scale on a hard floor. Weekly weighing is the worst of the options, because a single weekly reading carries all the day-to-day noise with no averaging to cancel it, and people then treat that noise as progress or failure.
Why did my weight go up overnight?
Because almost none of overnight weight change is fat. Storing a kilogram of fat requires an energy surplus of roughly 7,700 kilocalories, which no ordinary evening delivers. What moves overnight is water, glycogen and the physical contents of your digestive tract: every gram of glycogen stored binds around three grams of water, sodium shifts fluid within hours, and a large late meal is still inside you the next morning. Research that measured what two-week weight fluctuations are actually composed of found them to be roughly 84% fat-free mass. The correct response to an overnight rise is to record it and read the seven-day average instead.
How do I know if I am losing fat or muscle?
Read the weight and waist trend lines together rather than either alone. Weight falling while the waist stays put is the pattern that suggests a disproportionate share of the loss is lean tissue; the classic causes are too large a deficit, too little protein, or no resistance training. Weight falling with the waist falling alongside it is ordinary fat loss. Weight holding steady while the waist shrinks is recomposition, which is the best outcome available. Two supporting checks are worth running: whether your working weights in the gym are holding or climbing, and whether month-to-month photographs show the change the numbers are arguing about.
Do I need a DEXA scan?
Not for tracking change, which is what most people actually want. A tape measure and a scale used consistently produce trend lines that are more useful month to month than a scan taken twice a year, because the errors in a consistent home method stay still while the thing you are measuring moves. A single scan is genuinely worth buying if you want an absolute anchor: measure the same morning with your tape, note the offset, and apply it from then on. A scan is also the right tool (and a home method is not a substitute) when the question concerns regional lean mass, such as age-related muscle loss, recovery after injury, or monitoring during a medical condition.
