Waist-to-Height Ratio Calculator

Keep your waist under half your height. That single sentence carries more cardiometabolic screening power than BMI for many people, because it measures the fat that matters most: the visceral fat stored around your middle.

Divide waist by height (same units) and read the bands: under 0.4 may reflect underweight, 0.4–0.49 is the healthy zone, 0.5–0.59 signals increased central adiposity, 0.6 and above further increased. UK health guidance formally adopted this screen in 2022, and the 2025 obesity framework uses it as one of the confirmatory fat-distribution measures.

Most calculators stop at the ratio. This one also gives you the number you actually came for: the waist measurement that would put you exactly on the 0.5 line for your height, and how many centimetres, or inches, your tape currently sits from it. A ratio of 0.53 tells you very little on its own; "your waist is 5 cm above the line" tells you what to aim at.

These are the categories NICE set out in NG246 (January 2025) for adults with a BMI under 35: 0.4 to 0.49 healthy central adiposity, 0.5 to 0.59 increased, 0.6 or more further increased. NICE states they apply to both sexes and all ethnicities, including adults with high muscle mass, and summarises the advice as keeping your waist to less than half your height.

Known limitation: a single cut-off is slightly harsh on shorter adults and lenient on taller ones; a ratio a whisker over 0.5 matters less at 195 cm than at 155 cm.

What the waist-to-height ratio measures

The ratio answers one question: is your middle large for the frame it sits on? Waist circumference is a good proxy for visceral fat: the metabolically active fat packed around the abdominal organs, which drains free fatty acids straight into the liver and drives insulin resistance and inflammation in a way that fat on the hips and thighs does not. Height is the simplest available stand-in for frame size. Dividing one by the other turns a raw tape reading into something comparable between a 155 cm adult and a 195 cm one.

That is the gap it fills between the two measures most people already know. BMI sees total mass and cannot tell fat from muscle, or a wide waist from broad shoulders. Waist circumference on its own sees central fat but has no idea how big the body carrying it is, which is why its thresholds have to be published separately for men and women and adjusted by population. The ratio needs neither adjustment, because the height term does the scaling for you.

The formula, and why the units cancel

Waist-to-height ratio = waist ÷ height, with both measurements taken in the same unit. Because the unit appears on the top and the bottom of the fraction it cancels out completely: 87 cm ÷ 174 cm and 34.25 in ÷ 68.5 in both give 0.50. This is the reason the index travels so well between countries and why no conversion table is needed anywhere on this page.

It also means the ratio is dimensionless: it is not a length, a percentage or a score, just a proportion. The only arithmetic decision left is how many decimal places to report, and two is the sensible limit: measurement error in the waist alone is worth roughly ±0.01 in the ratio for an average adult, so the third decimal place is noise.

  • Both measurements in centimetres, or both in inches, never one of each.
  • Height is measured without shoes, standing tall, heels together, looking straight ahead.
  • A one-centimetre error in the waist moves the ratio by about 0.006 for a person of average height.

How to measure your waist properly

Stand relaxed, find the point midway between your lowest rib and the top of your hip bone (for most people that is at or just above the navel) and wrap a flexible tape level around it, checking in a mirror that it is not riding up at the back. Breathe out normally and read the tape at the end of that exhale, snug against the skin but not denting it. Measure two or three times and average. Consistency of technique matters more than millimetre precision. Waist circumference swings two to three centimetres through an ordinary day on food and fluid alone, which is more than a month of genuine fat loss usually shows, so measure first thing in the morning, after the bathroom and before eating, and compare like with like. Sucking in can shave three to five centimetres off the reading, the single most common way people fool themselves with a tape measure.

  • Same landmark, same time of day, same breath, every time.
  • Thin clothing or bare skin; a waistband under the tape adds a centimetre or more.
  • Two or three passes averaged, not a single hopeful reading.

The bands, and where they came from

The three boundaries at 0.4, 0.5 and 0.6 were first set pragmatically by Margaret Ashwell and Shiun Dong Hsieh, who argued in 2005 that a ratio was a faster and more universal public-health message than a table of waist thresholds. The evidence caught up with the pragmatism. In 2010 Browning, Hsieh and Ashwell reviewed 78 studies and found that waist-to-height ratio and waist circumference predicted cardiometabolic outcomes more often than BMI did, with mean areas under the ROC curve of 0.704 for the ratio, 0.693 for waist circumference and 0.671 for BMI. They concluded that 0.5 could serve as a global boundary value.

In 2016 Ashwell and Gibson tested the simple boundary against the more elaborate BMI-plus-waist "matrix" then used by Public Health England, using National Diet and Nutrition Survey data from 2008 to 2012. A single 0.5 line identified more people carrying early health risk than the matrix did: around a third of adults the matrix classified as fine had raised cardiometabolic risk factors on the ratio.

NICE adopted the scheme formally in guideline NG246, published in January 2025: 0.4 to 0.49 healthy central adiposity with no increased health risks, 0.5 to 0.59 increased central adiposity with increased health risks, and 0.6 or more high central adiposity with further increased health risks. NICE applies it to adults with a BMI under 35 and summarises the whole guideline in one sentence for patients: try to keep your waist to less than half your height.

Why one threshold works across sexes and ethnicities

Waist circumference thresholds have to be published separately for men and women, and the International Diabetes Federation publishes different ones again for South Asian, Chinese, Japanese and South and Central American populations, because the same tape reading carries different diabetes and cardiovascular risk in different groups. Much of that difference is a difference in body size rather than in biology: shorter populations reach the same relative central adiposity at a smaller absolute waist.

Dividing by height removes a large part of that. NICE states explicitly that the waist-to-height categories can be used for both sexes and all ethnicities, including adults with high muscle mass, the group BMI most reliably misreads, since a heavily muscled body carries its extra mass in the legs, back and shoulders rather than at the waistline. That is not a claim that risk is identical everywhere at a given ratio; it is a claim that one line is close enough to be useful everywhere, which is exactly what a screening tool needs to be.

The documented limitation at the extremes of height

The scaling is not perfect, and the people who study it say so. Waist circumference does not rise in a straight line with height across the adult range (allometric analyses put the height exponent well below 1, nearer 0.4 to 0.5), so dividing by height to the first power over-corrects for tall people and under-corrects for short ones. Nevill and colleagues made the argument directly in Obesity Research and Clinical Practice in 2023: a fixed 0.5 cut-off, as adopted by NICE and the US Department of Defense, will classify a systematically larger share of shorter adults as at risk and will be correspondingly lenient on taller ones.

In practice this matters at the margins, not in the middle. A ratio of 0.62 is a clear signal at any height; a ratio of 0.51 is a genuinely different situation at 155 cm and at 195 cm. If you are unusually short or unusually tall and your value sits within a couple of hundredths of a boundary, read it as "close to the line" and look at the waist-alone action levels and a second measure before drawing any conclusion.

What to do if you are over 0.5, and how fast a waist can realistically change

A ratio of 0.5 or more is a prompt to look further, not a diagnosis. The useful next steps are the unglamorous ones: a moderate calorie deficit planned from your maintenance calories rather than a crash, enough protein to hold onto muscle while fat comes off, resistance training two or three times a week, and, if the number is well over the line or you have a family history of type 2 diabetes or heart disease, a conversation with a GP, who can add the blood tests no tape measure can substitute for.

Set expectations honestly. Waist circumference falls roughly in proportion to fat loss, and a sustainable deficit produces something in the order of half a kilogram of fat a week, which for most adults shows up as somewhere around half to one centimetre off the waist per month once the first week's water shift has passed. Someone 5 cm above their 0.5 line is looking at a project of several months, not several weeks. Because that signal is smaller than the day-to-day swing in the measurement, judge progress on the direction of travel across three or four monthly readings taken the same way, never on a single week.

  • Re-measure monthly, in the same conditions, and average two or three passes.
  • Expect the ratio to move in hundredths, not tenths, from one month to the next.
  • A waist that stops falling while weight still drops usually means the deficit is now costing you muscle.

How it compares with BMI and waist-to-hip ratio

Against BMI, the ratio wins on what it can see and loses on what it has been validated against. BMI has decades of outcome data behind it and remains the entry point in every clinical guideline, but it cannot distinguish muscle from fat or central fat from peripheral fat, which is why the 2025 framework asks for a direct measure of fat distribution alongside it rather than instead of it. The ratio supplies that measure with a tape and no arithmetic beyond a division.

Against waist-to-hip ratio, the trade is simplicity for pattern information. Waist-to-hip needs a second circumference at the least reproducible landmark on the body, and it can flatter you for the wrong reason: gaining fat on the hips lowers the ratio exactly as effectively as losing it from the waist. Waist-to-height needs one circumference and a height you already know, and it cannot be gamed that way. Where waist-to-hip earns its keep is epidemiological pedigree: INTERHEART built its case for fat distribution over total mass on it, in 27,000 people across 52 countries. Use waist-to-height as the everyday screen, waist-to-hip when you specifically want to describe the pattern, and waist circumference alone when you want the number a clinician will recognise without conversion.

Dimensionless: the unit cancels, so centimetres and inches give the same answer. Report to two decimal places.

The 0.5 line: the waist that puts you on the boundary

Target waist = height × 0.5

The number this page exists to give you. For 174 cm that is 87.0 cm; for 5'8.5" that is 34.25 in.

The other two boundaries for your height

Waist at 0.4 = height × 0.4 · Waist at 0.6 = height × 0.6

Below the 0.4 waist the ratio enters the band that often reflects underweight; at or above the 0.6 waist it enters the further-increased band.

Worked example: an adult of 174 cm with a 92 cm waist

  1. Divide waist by height in the same unit: 92 ÷ 174 = 0.5287…, which reports as 0.53.
  2. Read the band: 0.53 falls in the NICE NG246 increased central adiposity band, which runs from 0.50 to 0.59.
  3. Find the 0.5 line for this height: 174 × 0.5 = 87.0 cm.
  4. Find the distance: 92 − 87 = 5.0 cm. That is the actionable number: the waist has to come down five centimetres to reach the boundary.
  5. Find the next boundary up, for context: 174 × 0.6 = 104.4 cm, so there is 12.4 cm of headroom before the further-increased band.
  6. Cross-check in imperial, to show the unit cancels: 92 cm is 36.2 in and 174 cm is 68.5 in; 36.2 ÷ 68.5 = 0.53, the same answer.
  7. Add the sex-specific screen: if this adult is a man, 92 cm is just under the WHO first action level of 94 cm, so the waist-alone test says 'not increased' while the ratio says 'increased'. The disagreement is height doing its work, and at 174 cm the ratio is the more informative of the two.

Frequently asked questions

Why is waist-to-height better than BMI for many people?

Because it measures where fat is, not just how much of everything you weigh. Visceral fat around the organs drives insulin resistance and cardiovascular risk far more than fat on the hips or muscle anywhere, and BMI cannot see the difference. Meta-analyses have repeatedly found waist-to-height ratio at least as good as, and often better than, BMI at predicting cardiometabolic outcomes, with one threshold that works across sexes and most heights.

What should my waist measurement be?

Under half your height is the memorable rule: below 87.5 cm for someone 175 cm tall, for example. This page works that number out for you and tells you how far your current measurement sits from it. Separately, WHO waist thresholds flag increased risk from 94 cm in men and 80 cm in women, and substantially increased risk from 102 cm and 88 cm. If your ratio is 0.5 or more, it is worth a conversation with a doctor regardless of what your BMI says.

Can my ratio be too low?

Below about 0.4 the ratio stops rewarding you: that band often reflects underweight, and the health risks of being underweight (bone density, immunity, hormonal function) are real even though public attention focuses on the other end. If you're under 0.4 alongside a BMI below 18.5, gaining to the healthy range is the evidence-aligned move, ideally with a clinician involved.

How is this different from waist-to-hip ratio?

Both target central fat, but they normalise differently: waist-to-height asks 'is your middle large for your frame height', waist-to-hip asks 'is fat concentrated at your middle rather than your hips'. Waist-to-height is simpler to measure well (hips have more landmark ambiguity) and works with one universal threshold. The 2025 framework accepts either as a confirmatory measure; we offer both calculators.

Does the 0.5 rule work if I am very short or very tall?

Less well than in the middle of the range, and this is the best-documented weakness of the index. Waist circumference does not scale with height in a straight line, so dividing by height over-corrects at the tall end and under-corrects at the short end. Nevill and colleagues argued in 2023 that the fixed cut-offs adopted by NICE and the US Department of Defense will classify a systematically larger share of shorter adults as at risk. Take a value close to a boundary as 'near the line' and look at a second measure before concluding anything.

How long does it take to move my waist-to-height ratio?

Longer than most people expect, because the ratio moves in hundredths. A sustainable calorie deficit takes off roughly half a kilogram of fat a week, which shows up as somewhere around half to one centimetre of waist per month for a typical adult once the first week's water loss has passed. Moving from 0.55 to 0.49 at a height of 175 cm means taking about ten centimetres off the waist, a project measured in months. Re-measure monthly, in the same conditions, and judge the direction of travel rather than any single reading.

Do I need to enter my sex to use this calculator?

No. The ratio uses the same 0.5 line for everyone, which is one of its main advantages over waist circumference thresholds, and NICE states that the categories apply to both sexes and all ethnicities. Supplying a sex on this page only adds the separate WHO waist-alone action levels (94 and 102 cm for men, 80 and 88 cm for women), which are sex-specific and are read against the same tape measurement. Leave it unspecified and you still get the complete ratio result.

Is waist-to-height ratio valid for children and teenagers?

The boundary values were developed mainly in adults, and the guidance that adopts them is written for adults: NICE NG246 covers people aged 18 and over. The simple message has been extended to children in some public-health materials, and a 0.5 boundary appears in paediatric research, but growth changes both terms of the ratio at different rates through puberty, so a single adult cut-off cannot carry the same meaning. Children and teenagers should be assessed with BMI-for-age percentile charts by a clinician who can put them in context.

Does a high ratio mean I have visceral fat specifically?

It raises the probability but does not prove it. Waist circumference correlates well with visceral adipose tissue at a population level, which is why it works as a screen, but at an individual level the tape cannot separate visceral fat from subcutaneous fat on the abdomen, from a large abdominal muscle mass, or from bloating and other transient causes. Imaging (CT or MRI, and to a lesser degree DEXA) is the only way to measure visceral fat directly, and it is neither cheap nor routine.
